MAC MEMBERSHIP PLANS

We’ve created a solution that allows your family to receive affordable and hassle-free dental  care  without  insurance. Unique membership plans provide comprehensive annual care solutions designed to fit your family and your budget.

Eligibility – Any patient without dental insurance is eligible to join. no pre-existing dental conditions will prevent the membership from being active. The patient account must have a zero balance membership fee and must be paid to remain active.

Exclusions– Coverage begins upon receipt of completed and signed contract. Membership extends for one year for each individual child from the date of purchase. Unused services do not carry over to new a membership contract. Membership fee is non-refundable and non-transferable

MAC Membership Agreement

Program Guidelines:
This Dental Plan is not insurance. The program is only offered to families not currently insured by a dental health plan. This plan is only valid at Dr. Bridget McAnthony’s office. The plan benefits are not transferrable to any other dental practice or provider. All payments are due at the time of service.

  • It is the member’s responsibility to schedule and keep all appointments offered as a part of the dental program.
  • Membership is for 1 year beginning on the enrollment date. Your membership will automatically renew on the anniversary date unless a written request to cancel is received 30 days prior to the renewal.
  • Membership dues are payable in full upon enrollment and are nonrefundable.
  • Payments for additional dental services are the member’s responsibility. Payment is due on the date of service to qualify for the discounts this member ship offers.
  • Membership must be current to receive the discount.
  • Fees for dental services may change at anytime.
  • Please notify our office at least 48 hours in advance if you must change a scheduled appointment.
  • No deductibles, no pre-authorizations, no yearly maximums and no waiting periods.
  • Discounts cannot be applied to any other specials or products such as fluoride, toothpaste and tooth brushes.
  1. MAC MEMBERSHIP PRIME AND MAC MEMBERSHIP SAFARI:  These are dental membership plans and are not to be considered dental insurance plans. The dental  membership  plan is for  patients without dental insurance  and cannot be used in conjunction with a dental insurance plan.
  2. ADMITTANCE TO THE PLAN: Members may only be admitted to the plan upon initial sign up or annually there after on the renewal date.
  3. USE OF PLAN AND SERVICES COVERED: This plan may be used at Dr. Bridget McAnthony’s office and applies only to dental procedures offered at Dr. McAnthony’s office.
  4. DENTAL CARE NEEDS TOO COMPLEX: There may be dental care needs beyond the scope of services provided by Dr. McAnthony. Patients with specialty dental care needs will be advised as soon as possible and referred to the appropriate dental specialists. Referred services are not covered by this membership plan. After specialty services are preformed the patient may return to Dr. McAnthony and receive treatment covered under the Membership Plan.
  5. PAYMENT OF MEMBERSHIP DUES: Membership dues are paid yearly and are due prior to services being rendered. Yearly dues may be paid with cash, check or credit card. Payment plans for membership dues are offered with AUTOMATED RECURRING BILLING ONLY. Dues must be paid in full and patient’s account must be current in order to receive benefits under the membership  If payment of dues has lapsed and patient does not want to or is unable to bring the account current, they must wait 12 months from the date of lapse to re-enroll in the plan.  Until that  time, they can continue to be seen as a patient and will be subject to normal fees.
  6. CANCELLATION OF MEMBERSHIP: Notification of cancellation of the plan must be made 30 days prior to the renewal date. There are no refunds for the subscription dues. Patients are not required to receive any recommended treatment that is diagnosed prior to using the benefits included with the membership.
  7. NON-TRANSFERABLE: This plan is non-transferable and can only be used by the registered members of the plan.
  8. PAYMENT FOR SERVICES: Payments for services are due at the time services are rendered.
  9. MEMBERSHIP DUES: Dues are subject to change annually. The dues are outlined as follows:
